Care Management and Counseling Services


Care management typically begins with an assessment of the senior adult, followed by the development of a Care Plan in cooperation with the senior adult and/or a family member. Resources for Seniors care management services are offered through several different programs.

Caregiver Counseling
RFS also provides one-on-one or family-based counseling for caregivers and families who are wrestling with how best to care for their aging loved ones. This fee-based service, provided by a licensed professional counselor, can assist with family conflicts over sharing care responsibilities or making placement decisions, sorting out options, and working through feelings of guilt, sadness, or anger.

Service Coordination
Resources for Seniors provides care management support services to residents of seniors’ apartments funded by public dollars or tax credits. Wake Independence At Home
A special care management service for senior and disabled adults who are clients of Wake County Human Services. Care management services include representative payee services, as well as funding for in-home aide services and adult day services.

Guardianship
Clients who are unable to manage their own affairs and who have not given someone power of attorney to act for them, may need a guardian to arrange care, manage finances, etc. A guardian may be a family member, an attorney, or someone employed by a public agency.
