Eligibility: Alzheimer's and related dementia patients
Fees: Enrollment is $55.00 plus $7.00 shipping & handling, which includes a MedicAlert medical ID. There is a $35 annual fee after the first year of membership.
Intake Process: $55.00 for set-up and initial payment

Program Description:
Provides a low-cost identity tag and registration in a national database for Alzheimer's patients. The cost is $55.00 (plus $7.00 S&H). This allows a centralized location for information in case the individual gets lost or wanders from home. Co-sponsored by MedicAlert Foundation and the Alzheimer's Association.
