Program Details

Hours: Monday-Friday, 8AM to 5PM
Eligibility: Medicaid eligible, using guidelines for clients living in private residences (i.e., not nursing home guidelines). Note, however, that eligibility is based on care-receiver's income only, not household income. Couples are allowed to separate assets to preserve some funds for the non-CAP spouse; consult Medicaid worker before doing this to avoid problems.
Fees: Services are funded by Medicaid. Some clients may be subject to a deductible which requires that they pay a portion of the cost of their care before Medicaid begins paying.
Intake Process: Call Intake Department at RFS to request placement on wait list.

Program Description:

Community Alternatives Program for Disabled Adults (CAP/ DA): Medicaid waiver program designed to help disabled adults receive care in a community setting, rather than in a nursing home. Provides case management, in-home care and a variety of other supportive services to enable the patient to remain at home. The goal is to offer a quality, cost-effective alternative to intermediate and skilled care nursing facilities. Eligibility: Must live in Wake County; be Medicaid eligible; have been deemed appropriate for intermediate or skilled nursing home care; and have health, safety and well-being needs that can be met in the home. Must also have a supportive caregiver or positive support from other significant people. This program does NOT provide 24-hour care. This program has a wait list of approximately 24 months at this time. Patients who have been in nursing homes since June 1, 2004 and wish to return to the community are given priority. ONLY patients who have been in NORTH CAROLINA nursing homes are eligible for this priority status. Patients moving here from out-of-state must go on the normal wait list. Applicants who are moving here from out-of-state may be placed on the wait list prior to their move. Once the process of applying for Medicaid begins, clients under the age of 65 must become resident in NC within 45 days of application; clients over age 65 must become resident within 90 days of application.
