Transitions LifeCare (founded as Hospice of Wake County in 1979) offers caregiver support, hospice, palliative, home health, and bereavement services. Transitions HospiceCare provides care to patients for whom cure is no longer an expectation, and who have a life expectancy of six months or less as determined by the physician. Primary care physician work in conjunction with the hospice care team. Services are provided regardless of age or diagnosis, and can be provided in the patient's home, long-term care facility, or retirement community. This area's only freestanding "hospice home" provides care when "home" is not an option. Care teams may includes nurses, social workers, hospice physicians, spiritual care counselors, chaplains, home health aides, pharmacists, bereavement counselors, volunteers, and other professionals as indicated. Other services included under the hospice benefit include medications related to the terminal illness, medical equipment such as hospital beds, and respite care. Transitions LifeCare opened Wake County's first freestanding inpatient hospice facility in January 2010, allowing patients under hospice care to remain in a home-like setting when they can no longer be cared for in their own homes. Transitions LifeCare also has a division called Transitions PalliativeCare, which provides medical and social work consultation for patients who have advanced illnesses and need pain and symptom management, but who may not be eligible for hospice services.
