Program Details
Eligibility: Homebound, requires skilled care, and has caregiver willing/ able to learn to assist when service ends.
Fees: Fees based on Medicare rates.
Intake Process: Must be referred by healthcare provider with orders for home care.

Program Description:
Medicare/ Medicaid-certified not-for-profit home health agency providing skilled nursing, PT, OT, ST, dietician, and social work. Provides home health aide only when skilled services are also being provided. NO private-duty home health aides, no PCS-only, no companions/ homemakers. When patient ceases to need skilled services, they are referred elsewhere if they have ongoing needs for home health aide. Accredited.
