Hours: Office hours: Monday-Friday, 8:30AM to 5:00PM
Eligibility: Medicare/ Medicaid-certified. Needs current FL-2 form filled out.

Program Description:
80 nursing home beds. Medicare/ Medicaid-certified. 20-bed short-term rehab hall. Offers short-term rehab, long-term care, assisted living, dementia care, bariatric care, etc. PT, OT, ST on staff. Also does wound vacuum, and IV therapy with antibiotics. Short-term respite is available on a case-by-case basis.
