Eligibility: Takes all patients with a payor source. Hospice patients must have limited life expectancy.

Intake Process: Intake available 24/7. Call to make initial contact or request referral from physician.
Service Area: Wake, Johnston, and 15 neighboring counties in eastern North Carolina
Program Description:
Medicare/ Medicaid-certified home health and hospice agency, providing skilled nursing, home health aides (only if skilled care is being provided concurrently), private-duty nursing, and therapies including PT, ST, OT. Can provide incontinence products through Medicaid. Accredited by Community Health Accreditation Program (CHAP). Inpatient hospice facilities located in Goldsboro and Newport, NC.
