Program Description:
Patients seeking Medicaid-funded Consolidated Personal Care Services (PCS) must undergo an "independent assessment" by CCME. The assessment must be requested by the patient's physician. Physicians must fax or complete referral form online (www.thecaroinascenter.org/OPRS). Patients seeking to change providers or to get more hours due to a change in health status must also request this through CCME. Forms can be found on website; look for PCS Independent Assessment.
