Fee-based care management practice, operated by an RN, offering services including coordination, planning, assessment, and continuing care for seniors. Can accompany clients to physician appointments and report back to family. Also reviews and manages medical bills to avoid overcharges and duplication. Medication management services include setting up pillboxes, verifying prescriptions, arranging refills, and daily telephone calls to check on compliance. Can coordinate selection of long-term care facility and assist with arranging packing and move-in.
