Program Description:
(Out of Business) Aging Life Care Management service, within a home care agency. Advocacy, assessments, coordinate and monitor resources, crisis intervention, one time consultations, development and implementation of care plan, facility evaluations and tours, facilitate discussions between the senior, family, and professionals, accompany to medical appointments, friendly visits, supportive counseling.
