Program Details
Hours: Monday-Friday, 9AM to 6PM for inquiries
Eligibility: Age 55+ (though will consider a little younger if over 50) and ambulatory.
Fees: Accept Special Assistance. Private pay rate: $1700 for shared room; $2500 for private room.
Intake Process: Get FL-2 form, then call to inquire about availability. 2-step TB test required within the last 90 days. Medicaid applicants must apply for Special Assistance prior to admittance.
