Program Description:

PACE is a program offering a comprehensive set of healthcare and support services for older adults. With funding from Medicare, Medicaid, and other sources, PACE programs provide medical care, skilled therapies, home health care, adult day care, and other services in a coordinated, managed care model. To be eligible, participants must be at least 55 years old, and must need a level of care equivalent to that provided by a nursing home, but must be capable of living safely at home with the support of the PACE services at the time of enrollment. If the PACE participant eventually needs nursing home care or hospitalization, the PACE program pays for it and continues to coordinate care. The program's facility is located in RTP and will serve Wake and Durham counties with a capacity of 300 patients.
