SCCNC provides the PACE (Program of All-Inclusive Care for the Elderly) program for all of Wake and Durham counties as well as select zip codes in Granville county. PACE is regulated by Centers for Medicare and Medicaid Services (CMS) as well as NC Division of Medical Assistance (DMA). PACE is a comprehensive managed health care and support services program for people over 55 who meet nursing home level of care by state requirements, but are able to live safely in the community with PACE. PACE services can include adult day health, transportation to and from participant's homes, medical and specialty care, nursing care, prescription drug coverage, medical equipment and supplies, support with meals, therapy services, respite care, personal care, dental and vision care, hearing care, nursing home care, hospitalizations and more with authorization by the interdisciplinary team. Services are provided at the PACE center, in participant's homes and in the community.
