Program Description:
As of October 1, 2013, this is the Independent Assessment Entity (IAE) vendor for the Division of Medical Assistance (DMA) to conduct Medicaid PCS Assessments. They handle all requests for new referrals and change of status. Physicians should fax or mail the completed "Personal Care Services (PCS) Request for Services Form" which can be found at: http://info.dhhs.state.nc.us/olm/forms/dma/dma-3051-ia.pdf Please Note: If a "Request for Services Form" is submitted incomplete, an "unable to process" notification will be issued and a new request for services form will be required.
