Help families navigate the challenges that normally come with aging and with chronic disease, including acting as a transition specialist from hospital to rehab or to home or facility, accompanying the client to medical appointments, communicating health information to client's family, and helping the client to understand and implement treatment plans. Provide referrals to needed services (legal, financial, home modification, therapists, home health, facilities, senior movers, etc.). Also provide long distance caregiving. Advocates are social workers, RNs, and Masters in Public Health or Counselor Education. Carries business and professional liability insurance. Member of, and credentialed by, professional organizations: APHA (Alliance of Professional Health Advocates), NAHAC (National Association of Health Advocacy Consultants), and both the National Aging In Place Council and its Triangle Chapter.
