Program Description:
Geriatric care management services, fee-based, for families or caregivers of seniors. Includes assessment, care planning, care coordination, screening and placing caregivers, care monitoring, crisis intervention, counseling and support, long distance reassurance, information and referral, advocacy, assistance with placement, and assistance moving in and out of the Triangle. One-time consultation and ongoing care management are both available. Also offer guardianship services. Can provide "check-in" services to monitor an older adult's wellbeing while caregiver is out of town and respond to emergencies if needed. Also runs Senior Choices, which is the same care planning service, but designed for the seniors themselves.
