Program Description:
Care management and counseling practice including nurses and social worker with geriatric experience. Provides care planning assessments, arranging and monitoring of services, organizing medical care, crisis intervention, assisting with moves to a new setting, psychotherapy/ counseling (Medicare-certified, can do home visits), family interventions around eldercare, training and education.
