Program Description:
Fee-based patient advocacy, case management, and nursing service providing a variety of services including: *Regularly-scheduled visits to seniors in order to support self-management such as accurate medication, exercise, and assessment of medical conditions. *RN accompaniment to physician visits to assist with communication and understanding of changes made by physicians. Act as liaison with multiple physicians. *Assessment of senior's needs and development of care plan options and guidance for decision-making. *Nursing treatments such as dressing changes, catheter changes, colostomy care, injections, G-tubes. *Patient/ Family education on disease processes and management (e.g. dementia). *Act as liaison for family with physicians, facilities and support services.
