Program Description:
NOTE: No longer operates toll free line.
Program provides individualized "medication therapy management" services for seniors who are enrolled in a Medicare Part D plan. A pharmacist meets one-on-one with the senior to review their medications and make sure they are using the medications as directed. They also look for possible duplications, unnecessary medications, or harmful side effects and interactions. They then communicate both with the senior and their doctor regarding any changes that need to be made, and can follow up to make sure the changes were effective. In addition, the pharmacist can help the senior determine whether their Medicare D plan covers their medications, and whether a lower-tier/ less-expensive alternative might be available.
There is no cost to the senior for this service, and eligible seniors can receive the service once per year. If a family caregiver manages the senior's medications, the meeting can be held with the caregiver.
To locate a ChecKmeds NC Pharmacist in their area, seniors can call the local number or go online to 'www.getoutcomes.com' to search by zip code. Resources for Seniors is providing this service through our MEDS program, including offering services in the client's home if needed. Many chain pharmacies like Kerr Drug, Kroger, Walgreens and others, as well as independent pharmacies, are also offering this service for their customers through in-store consultations.
ChecKmeds NC is funded by the NC Health and Wellness Trust Fund and implemented through the NC Office of Rural Health and Community Care.
