Transitions HomeHealth, a division of Transitions LifeCare (formerly Hospice of Wake County), serves patients with life-threatening illnesses who may be receiving aggressive treatments for the illness. Patients do not have to be terminally ill or receiving only palliative care. Services available: skilled nursing, home health aide, social work, physical therapy, occupational therapy, speech therapy, and spiritual care counselors.
