Program Details
Eligibility: Serves anyone who is homebound, needs skilled nursing services, and has a personal attending physician.
Fees: Accepts Medicare, Medicaid, most private insurers, and private pay. However, does not deny care to anyone because of inability to pay.
Intake Process: Call 919-828-0890 and ask for Intake department.

Program Description:
Transitions HomeHealth, a division of Transitions LifeCare (formerly Hospice of Wake County), serves patients with life-threatening illnesses who may be receiving aggressive treatments for the illness. Patients do not have to be terminally ill or receiving only palliative care. Services available: skilled nursing, home health aide, social work, physical therapy, occupational therapy, speech therapy, and spiritual care counselors. Transitions HomeHealth is a non-profit home health care agency.
