Do Not Resuscitate (DNR), and Medical Orders Scope of Treatment (MOST) forms are signed by a patient's physician and posted prominently in the patient's room (or carried with the patient, if travelling), to signal emergency personnel NOT to perform cardiopulmonary resuscitation or other patient wishes, in the event of cardiac arrest or pulmonary failure. These orders are generally used when the patient is terminally ill or severely demented, and CPR would be considered an extraordinary measure. These forms can be obtained from the Office of Emergency Medical Services by a doctor, hospital, or health care facility. These are the only forms that would legally allow EMS personnel to honor the patient's wishes for limited treatment -- a living will is not effective in an emergency situation.
