Intake Process: See website for instructions to register. When registering directives, please be sure to include registration form, payment for processing, and check that the notary seal is dark and legible.

Program Description:

State-sponsored program allows healthcare powers of attorney, living wills, organ donor requests, and mental health powers of attorney to be registered and made available online, 24 hours a day. People who register their documents receive an access code which may be used by their doctors, family members, etc. to retrieve their documents as needed. See website for details. Website has legal forms that can be downloaded for healthcare powers of attorney, living wills, etc. These forms are available at no cost and can be used whether or not you register them.
