Hours: Monday-Friday, 7AM to 9PM; weekends by appointment
Eligibility:
Fees: $50 per hour

Program Description:
Care manager in private practice offering in-home assessment, coordination of services, family consultations, assistance with selecting a long-term care facility, etc. Background in home care, medical experience in hospital and assisted living settings, with MS degree in gerontology.
