Hours: Mon-Fri 8-6, Sat and Sun by appt
Eligibility:
Fees: Accepts Medicare but not Medicaid, and does take private insurance. Home safety evaluation is covered by Medicare if they are providing OT. Private rate is about $75/hr.
Intake Process: Call to request services. Physician referral is not required for the home safety evaluation, but will be needed if Medicare is being billed.

Program Description:
Occupational therapy practice offering clinic-based OT services as well as home safety evaluations. Also has fall prevention, diabetes management, fitness, and ADL skill training. Medicare-approved. Medicare may cover Home Safety Evaluation based on need and referral from a physician.
